How to Apply

Support Services

Application for Support Services can be requested by e-mail, phone or fax.  For Support Services, documentation of disability and other records are required as part of the application process, along with an assessment of Adult Living Skills.  An interview is required with the individual, parent(s), guardian, or referral source.  Additional information may be requested in order to determine eligibility.

Club Esprit & Players

Application for membership in Club Esprit (with or without Esprit Players) can be requested by e-mail, phone or fax. The application process requires a simple application form, interview with the applicant, and collaboration with the family member, guardian or other referring program staff. 

Request for State Funding Assistance

Any applicant who is  seeking funding assistance for Esprit services from the Massachusetts Department of Developmental Disabilities should contact their current DDS Services Coordinator or Esprit ( if not currently a DDS consumer).
